Please note: After scheduling your treatment, you will need to complete your online intake paperwork prior to your appointment. This paperwork will be sent to you via email and should be done at least 48 hours prior to your appointment. This intake form is to ensure you have medical clearance prior to your appointment and is vital for your safety. The form should take about 5 minutes to complete, and only needs to be completed before your first appointment or if you have any significant changes to your medical history.
